Steps to Certification

On October 30, 2020, MSNCB transferred the Care Coordination and Transition Management (CCCTM) certification program to the American Nurses Credentialing Center (ANCC).

If you hold an active CCCTM certification and are renewing, are interested in the CCCTM certification, or looking to verify CCCTM credentials, please visit the ANCC website. You may also contact ANCC at for more information.

1. Verify your Eligibility

nursesThe first step in preparing for your exam is to review the eligibility criteria.

2. Review the Exam Blueprint

Find out the contents of the exam and what you’ll be tested on. The exam blueprint can also help you prepare for the exam because it breaks down the sections of the test.

3. Complete the Exam Application

Be sure you enter your name on your application exactly as it appears on your unexpired, government issued photo ID.

About 3-5 days after submitting your application, MSNCB will email your Authorization to Test or contact you for more information.

4. Start Studying

There is no reason to wait to start preparing for the exam! There are several excellent study resources available to you for the Certified in Care Coordination and Transition Management (CCCTM®) exam. They vary on material, learning styles, and cost. Some are even free!

5. Schedule your Exam

Your ATT will list the 90-day period within which you have to take your exam. Confirm that your name is listed on your ATT exactly as it appears on your unexpired, government issued photo ID. If they do not match, contact MSNCB at 1-866-877-2676.

The ATT will also contain information about how to schedule your exam online. Schedule your appointment as soon as possible, even if you plan to test toward the end of your testing period. Seats can fill up quickly and are on a first-come, first-served basis.
