CCTM Certification Volunteers Making it Possible

MSNCB has developed a certification for care coordination and transition management (CCTM) to be offered September 2015. This important work could not be done without nurses in the CCTM role from both acute and ambulatory care settings contributing their time and talent. We would like to introduce you to the volunteers who worked diligently to develop the Certified in Care Coordination and Transition Management (CCCTM®) certification exam.
