MSNCB Congratulates First RNs Certified in Care Coordination and Transition Management [1]

MSNCB has announced the names [2] of the first nurses to earn the Certified in Care Coordination and Transition Management (CCCTM®) [3] credential. Along with 25,000 CMSRNs, the new CCCTM nurses are part of the MSNCB community.

MNSCB now has two important credentials in our vibrant community of proud nurses who have validated their expertise in their respective practices,” said MSNCB President Mimi Haskins, DNP, RN, CNS, CMSRN.

The CCCTM exam, which was launched by MSNCB September 1, was a collaboration of MSNCB and the American Academy of Ambulatory Care Nursing [4] (AAACN). AAACN contributed additional leadership and expertise for the new certification, along with study resources.

According to Haskins, the new certification will help ensure patients don't fall through the cracks in the health care system because they have an expert nurse guiding them through.

"Nurses have been coordinating care and managing transitions for decades, however the need for -- and complexity of -- care coordination has increased dramatically in recent years," Haskins said.

The CCCTM credential validates the knowledge and expertise of RNs in the CCTM role. Thanks to the knowledge and skills of CCCTM nurses, patients and facilities will receive better coordinated and more cost-effective care, including preventive care and health promotion strategies.

The exam is administered by MSNCB and is based on AAACN's Care Coordination and Transition Management Core Curriculum [5]. RNs who have a minimum of 2 years' experience in a CCTM role and at least 2,000 hours of CCTM practice within the last 3 years are eligible.

With the pressure on health care facilities to achieve better outcomes intensifying, CCCTM nurses will be in great demand and will have a competitive edge in today's job market, Haskins said.

Background
The focus on CCTM began several years ago when AAACN leaders Sheila A. Haas, PhD, RN, FAAN, Beth Ann Swan, PhD, CRNP, FAAN, and Traci S. Haynes, MSN, RN, BA, CEN, identified that the RN role in care coordination was desperately needed in all health care settings – from acute care to ambulatory care and everything in between.

The organization started the care coordination ball rolling by producing the Core Curriculum [5] as well as core competencies and a 13 module online course. The AAACN leaders then reached out to MSNCB's experts and the CCCTM exam was born.
