AMSN was represented at the recent AONE Conference in Indianapolis, IN. AMSN member Leslie Gervase and American Academy of Ambulatory Care Nursing (AAACN) member Gregory Kopp presented the work of the AMSN-AAACN Care Transition Hand-off Tool Task Force, a joint collaboration between the two associations. The toolkit is a free online resource designed to help nurses achieve optimal patient outcomes.
